The A to G assessment is an invaluable tool and a regularly used way of assessing patients who are deteriorating or who are at risk of deteriorating.

Circulation
Exposure
Fluid Balance
Mucous Membranes
Look for dry tongue / oral cavity
Skin Turgor
Pinch a fold of skin and look for delayed return to normal contour
Jugular Venous Pressure (JVP)
Measure (<4cm above the sternal angle at 45 degrees)
Oedema
Pitting vs. non-pitting - apply pressure for 15 sec
Urine Output
